Pit and Fissure Sealants

The chewing surfaces of teeth are never flat. They have infact certain depressions called Pit and Fissures which serve as potential traps for food and bacteria making the teeth susceptible for decay. Although other factors such as dietary habits, oral hygiene and amount of sugar intake do pay an important role but the pits and fissures have been suggested as " the single most important anatomic feature leading to the development of tooth decay". THEREFORE as a preventive measure ,these pit and fissures must be sealed with a material so that there is no space where food can stay. This is done with Pit and Fissure Sealants.

Sealants are a plastic material applied on the chewing surfaces of the posterior teeth which help shield out decay causing bacteria by filling the fissures and grooves on the chewing surfaces

FAQ About Sealents

1. Is sealant application a complicated procedure?

For a good professional , sealants are easy to apply. It takes 5 to 10 min to seal a tooth.

2. Do sealants need to be reapplied?

Sealants usually stay well under normal chewing forces , but recall of patients is necessary to ensure that sealant is in place and is not lost either fully or partially. If required , sealants should be replaced.

3. Are sealants for kids only?

Sealants are recommended for the children who are prone to tooth decay . In these children , sealants are applied immediately or as early as possible after the tooth erupts in the mouth. But adults can benefit from sealants as well.

These are contraindicated in cases of already existing decay which require fillings.

Finally, although sealant application is relatively simple, the meticulous technique requires patient cooperation and should be postponed for uncooperative patients until the procedure can be properly executed.

TYPES OF PRR

Three types depending upon extent and depth of caries lesion

  1. Type –A
  2. Type –B
  3. Type –C

Type A Preventive Resin Restoration

Comprises of suspicious pits and fissures where caries is limited to enamel. A slow speed round bur is used to remove any decalcified enamel

Steps involved

  1. surface is cleaned
  2. tooth is isolated preferably with rubber dam
  3. decalcified pits and fissure are removed with a slow speed round bur
  4. acid etching gel is placed over the entire occlusal surface for 60 sec.
  5. surface is then washed for 20 sec. , and dried for 10 sec.
  6. sealant is applied carefully , avoiding air entrapment in the preparation site
  7. polymerization done with visible light for 20 sec.
  8. occlusion is adjusted ,if needed with finishing bur

Type –B Preventive Resin Restoration

comprises of incipient carious lesion extending into dentin that is small and confined.

Steps involved

  1. thorough prophylaxis of the surface
  2. isolation of the tooth
  3. caries from pit and fissures removed with slow speed round bur
  4. etching gel placed over the entire occlusal surface for 60 sec.
  5. wash for 20 sec . and dry for 10 sec.
  6. coat of bonding agent applied on the walls of preparation
  7. preparation then filled with composite material
  8. filled sealant material is applied over the entire occlusal surface
  9. all layers are simultaneously cured
  10. occlusion is adjusted
  11. surface is finished and polished

Type –C Preventive Resin Restoration

Is characterized by the presence of deep caries and need for greater exploratory preparation in dentin

Steps involved

  1. thorough prophylaxis of the surface
  2. isolation of the tooth
  3. caries from pit and fissures removed with slow speed round bur . In case of pain , anesthesia may be given
  4. 4. cavo surface margins are beveled
  5. base of fast setting Ca hydroxide is given to cover the exposed dentin surface
  6. etching gel placed over the entire occlusal surface for 60 sec.
  7. wash for 20 sec . and dry for 10 sec.
  8. coat of bonding agent applied on the walls of preparation
  9. preparation then filled with composite material
  10. filled sealant material is applied over the entire occlusal surface
  11. all layers are simultaneously cured
  12. occlusion is adjusted
  13. surface is finished and polished

Space Maintainers

A space maintainer is a removable or fixed appliance designed to maintain the space created by early loss of milk teeth till the time the permanent tooth erupts in the space. If this space is not maintained , this space would likely to be reduced and later the space may not be sufficient for the permanent tooth to erupt properly.

Root Canal Treatment in milk teeth

Indications and procedure is same as for permanent teeth with slight difference . Although the morphology of milk teeth makes the treatment difficult, it might still be considered as a better alternative to tooth extraction.
